Dental Insurance and Payment Options
Please note: Payment is due at the time of service.
As of January 1st, 2024, we will no longer accept cash payments. Accepted forms of payment will be credit card, check, debit card or cashier’s check.
Our office is not contracted with any insurance plans. You will be utilizing out-of-network benefits with our office. As a courtesy to you, we will send claims to your insurance company and track claims. There are certain insurance plans that we cannot send claims to. Please keep us informed of any changes to your insurance plan and make sure you provide us with accurate plan information. You are responsible for the fees charged by our office, no matter what your insurance coverage may be. Most insurance companies should respond to the claim within four to eight weeks. estimates for treatment
We give full treatment plan estimates the day of your consultation appointment. This gives you a chance to sit down with our financial coordinator who will answer any questions that you have about the financial part of your care. Our goal is always to be 100% transparent with any fees we anticipate you will owe. For an idea of treatment plan estimate ranges, please see below.
Wisdom teeth: As of November 1st, 2023, for patients under the age of 25 years, there is a bundled fee that will not exceed $2500 for the consultation, any necessary imaging, extraction of impacted and/or erupted teeth, and anesthesia. Any additional procedures such as biopsies, extraction of additional teeth, surgical exposure of additional teeth, implants, bone grafts, etc., would be itemized and treatment planned at their usual fees. Treatment plans with bundled fees do not qualify for a 5% discount.
For patients over the age of 25 years, a treatment plan will be provided including our standard, itemized, non-bundled fees. We have elected this breakdown as patient age is the primary factor determining the complexity of the procedure, not whether the teeth are impacted or not.
Dental implants: Fees can range between $3385-4955 for a single, custom-guided simple dental implant including exam, imaging and sedation fees.
Single tooth extraction: Fees can range between $460-1110 for a single tooth extraction under local anesthesia including exam and periapical xray.

Insurance
Prior to your appointment, please provide your dental and medical insurance information to the staff. We are not in-network providers with any dental or medical insurance companies. Fees are collected at the time of service for consultation appointments as well as procedures. We strive to be as transparent as possible with our fees and policies. Depending on the insurance company, they may reimburse you directly instead of sending payment to our office. Once a claim is submitted, the insurance company makes a final decision to determine what the payment will be. Your insurance is a contract between you or your employer and a dental insurance company. The benefits you receive from insurance are based on the terms of that contract. The goal of most insurance policies is to provide only basic care for specific services. The benefits that you receive from the dental insurance company under your current insurance plan have nothing to do with you personally or achieving a high quality, comprehensive result. Our office will do everything possible to help you understand and make the most of your dental insurance benefits. As a courtesy, our office will complete and submit your insurance claims to achieve the maximum reimbursement to which you are entitled. Please remember that you are ultimately responsible for all expenses incurred. We urge you to read your insurance policy so that you are fully aware of coverage and any limitations of the benefits provided. There are several insurance plans that we cannot submit claims to on your behalf. * If you are unsure of whether you may receive care from an out-of-network provider, please contact your insurance carrier to request that information.
*We are not contracted providers with Caresource, Molina, Paramount, Medicare, or any other government funded insurance options and anyone that chooses to see one of our providers that carry those insurances will be self-pay for services rendered.

confirmation/cancellation/late policy
We make every attempt to contact you ahead of your scheduled appointments for confirmation. Please ensure that your contact information is up to date and accurate with our office. For consultation and regular surgical procedure appointments we require a 48-hour confirmation or cancellation. For implant procedures and any surgical procedures scheduled 90 minutes or longer, we require a 5-day confirmation or cancellation. If you have any questions about our cancellation policy, please call the office. You are required to arrive at the office 15 minutes prior to your scheduled appointment to check-in with all necessary paperwork completed. In respect to other patients scheduled, if you arrive late for your appointment or do not have the necessary paperwork completed at the time of your appointment, we may need to reschedule it to a different day.
